<p>The coexistence of an abscess inside brain metastasis is a peculiar pathology. The preoperative radiological diagnosis is not always conclusive. The management includes gross total resection followed by antibiotics and palliative therapy. A 46-year-old man with a history of locally advanced nasal squamous cell carcinoma presented with left-sided weakness and brain imaging showed a right frontal mass. The patient underwent surgical excision, and although a frank pus was found intraoperatively, the histopathological examination confirmed the presence of metastasis. We report this rare case of an intracranial metastatic squamous cell carcinoma with an abscess inside it, and we reviewed the literature for other cases of this rare entity. The diagnosis of abscess inside metastasis is challenging. The intraoperative presence of frank pus inside brain lesions should not preclude the diagnosis of tumors especially metastasis, particularly in patients known to have primary cancer. Neurosurgeons should be aware of this rare pathology owing to the different treatment plans.</p>
