<p>Organophosphate-induced delayed neuropathy (OPIDN) is a neurotoxic disorder characterized by both peripheral axonal degeneration and central (pyramidal tract) involvement with long-term impairments such as spasticity and weakness. This case report describes a 73-year-old male with delayed-onset OPIDN following chlorpyrifos exposure. Initially diagnosed with sepsis, he later developed motor neuropathy and spasticity. Nerve conduction studies confirmed motor neuropathy. While symptomatic treatment led to slow improvement, spasticity persisted. This case underscores the unusual and insidious development of OPIDN following exposure to chlorpyrifos, notably in the absence of any prior acute toxic symptoms and highlights the need for heightened clinical vigilance in cases of pesticide exposure, even when initial symptoms are absent, as the delayed progression of neurotoxic effects may obscure the diagnosis and impact timely intervention.</p>
