<p>Post-auricular dermoid cysts often present as painless, mobile, cystic swellings that are usually mistaken for lipomas or hemangiomas. They are typically benign and can occur anywhere but are rare in the post-auricular area. We report the case of a 22-year-old male with congenital right-sided postauricular swelling. Physical examination revealed a 3 × 2&#xa0;cm non-tender, non-mobile, and non-compressible cystic mass with normal skin. Computed tomography showed a superficial subcutaneous cyst measuring 2.5 × 2&#xa0;cm, without intracranial communication or extensions. Surgical excision revealed a lumen containing hair and two tooth-like structures. Histopathology confirmed the presence of a dermoid cyst with stratified squamous and transitional epithelium, hair, and bone formation. The postoperative course was uneventful, and no recurrence was observed at the 1-year follow-up. Post-auricular dermoid cysts are rare, usually asymptomatic, and often treated for cosmetic reasons. Imaging is crucial for diagnosis and surgical planning, and histopathological examination is required to exclude malignancy. Complete excision is vital to prevent recurrence.</p>
