<p><i>Cinnamomum balansae</i> is grown in plantations in northern Vietnam for its fine timber and essential oils. However, plantations are vulnerable to diseases. Our study recorded canker and wilt diseases with symptoms of stem cankers, discolored bark and wood, wilting canopies, and tree death in plantations. The 16-year-old trees in plantations were seriously damaged with disease incidence of 30.2%. ITS, β-tubulin, and EF1-α gene sequencing confirmed that the pathogen causing canker and wilt diseases in <i>C. balansae</i> in Hoa Binh province, Vietnam was <i>Ceratocystis fimbriata</i>. Strains of the pathogen had strong pathogenicity on <i>C. balansae</i> seedlings in an inoculation trial in a nursery. There were significant provenance differences in disease tolerance, with the Thanh Hoa provenance having the lowest lesion length (L = 2.25&#xa0;cm), mortality (D = 6.7%) and the highest tolerance to <i>C. fimbriata</i>. The Phu Tho provenance and commercial seedlings were susceptible to the pathogen with L = 14.6–17.1&#xa0;cm, D = 46.7–63.3% and had very low disease tolerance. The tolerant provenance provides a scientific basis for the selection of disease-tolerant varieties in the future. Furthermore, to minimize damage to existing <i>C. balansae</i> plantations, appropriate management solutions need to be identified and implemented.</p>
