Pessartherapie – Anpassung in der gynäkologischen Praxis

Vaginal pessary therapy is an established and guideline-recommended conservative treatment option for women with symptomatic pelvic organ prolapse. International guidelines recommend offering conservative treatment to all symptomatic patients, regardless of prolapse stage. This review aims to provide a practical overview of pessary fitting, appropriate pessary selection, follow-up strategies and predictors of successful long-term use. Current evidence demonstrates significant improvements in prolapse-related symptoms and quality of life following pessary treatment. Long-term adherence depends largely on structured fitting procedures, realistic patient counselling and promotion of self-management. In addition, evidence-based selection of lubricants is discussed, as lubricant composition may affect vaginal mucosal integrity and treatment acceptance.
