摘要

Gastric antral vascular ectasia (GAVE) accounts for about 4% of all nonvariceal upper gastrointestinal bleeding. Classic manifestation is in the form of a watermelon stomach or vascular angioectasia radiating from the pyloric ring to the gastric body. The disease is associated with liver cirrhosis, but it can also occur in patients with autoimmune diseases, heart valve defects, or renal failure. Diagnosis is made based on gastroscopy. In cases of uncertainty, a biopsy can be used to confirm the diagnosis. Therapy should be reserved for patients with iron-deficiency anemia or acute upper gastrointestinal bleeding. Endoscopic therapy is considered the gold standard, while drug therapy and surgical resection of the stomach are rarely used. The most common endoscopic treatments include argon plasma coagulation, radiofrequency ablation, and band ligation. In German-speaking regions, the most common form of treatment is argon plasma coagulation. A recently published meta-analysis demonstrated a higher eradication rate for the disease, lower recurrence rate, and lower costs for endoscopic band ligation. This new knowledge should be incorporated into future treatment approaches. Radiofrequency ablation is generally used as rescue therapy when other treatments have failed.
