<p>The simultaneous pancreas-kidney transplantation (SPK) is the gold standard for selected type 1 diabetes patients with terminal kidney failure. It combines the restoration of renal function with a permanent normoglycemia, which results in the long-term freedom of patients from dialysis and exogenous insulin treatment and a clear reduction in delayed cardiovascular and microangiopathic damage. Long-term data confirm a 1‑year patient survival of over 95% and a transplant survival of around 76% after 5 years. Despite this success the mean waiting time in Austria up to 2024 has more than doubled to 17.6 months, which is due to an aging donor pool and a declining surgical routine. These delays threaten the predialysis transplantation window and worsen the outcome of numerous patients.</p>
