Patient and Provider Insights on the Design and Engagement of Digital Health Recommendation Systems

As digital health technologies such as apps proliferate, recommendation services or portals are increasingly being used by clinicians and patients seeking to find useful tools. The design of such systems matters, as it plays a role in enabling the informed selection of health apps appropriate to individual needs and interests. The objective of this study is to identify points for consideration in the design of the M-Health Index and Navigation Database (MIND), as revealed through feedback provided by patients and providers. As MIND is one of the largest ongoing digital health recommender systems, insights from the study provide broad implications and inform practical guidance for the design of similar systems. The method used in this study followed a qualitative, interview-based design, with interview sessions conducted with patients and providers. A thematic coding analysis of interviewee comments was used to generate results under each of three dimensions of design informed by the literature, namely aesthetics/visual design, user experience, and presentation and design of information. Under the first dimension of design, color mattered in fostering an approachable experience. However, perceptions of color in the display differed between patients and providers. User experience design also mattered, with usability supported through a clear, uncluttered, and simple organization and layout, challenged in particular ways for first time users, and enabled with a robust search function. Finally, the presentation and design of the app information mattered, to the extent its design accommodated the interests of users to make easy comparisons among apps. Design matters in creating user-friendly, accessible, engaging, and customizable digital health recommendation systems. Attention to the use of flexible visual design options, incorporating interactive and user-specific onboarding experiences, and the selection of information presentation formats that enable easy comparison without overwhelming users can contribute to a well-designed digital health recommendation system that responds to patient and provider interests and needs.
