<p>Immunoglobulin G4-related disease (IgG4-RD) is a rare immune-mediated fibroinflammatory condition that can affect multiple organs, though breast involvement is exceptionally uncommon. We report the case of a 41-year-old woman presenting with a left breast lump clinically and radiologically suspicious for malignancy. Core biopsy revealed lobular mastitis, and immunohistochemistry showed an elevated IgG4/IgG plasma cell ratio (&gt; 40%) accompanied by raised serum IgG4 levels. A final diagnosis of IgG4-related sclerosing mastitis was confirmed following lumpectomy. This case underscores the importance of considering IgG4-RD in the differential diagnosis of breast masses, as early recognition can guide appropriate medical management and avoid unnecessary surgery.</p>
