PD-L1 expression in Ovarian Carcinoma and its Correlation with Clinico—Pathological Features: An Experience at a Tertiary Care Hospital in North India

Ovarian carcinoma is one of the most common gynaecological malignancies exhibiting a diverse histopathological pattern. Despite the standard treatment modality, associated mortality is high. A complete understanding of the clinical pathological features, tumour micro environment, and the expression of bio markers is necessary for framing novel immunotherapy methods aimed to improve patient survival. Program death ligand—1 (PD-L1) is a protein expressed on the surface of tumour cells which inhibits the anti tumour activity of the immune cells in the tumour micro environment. Hence, a study of the pattern of expression of PD-L1 on the tumour cells in ovarian carcinoma can enable the use of PDL1 inhibitors as an adjunct in the treatment of ovarian carcinoma.
MethodsPDL1 expression was evaluated by immuno histochemistry in 55 cases of ovarian carcinoma and its expression was correlated with various clinical and pathological features.
ResultsPDL1 expression was noted in 25.45% cases of ovarian carcinoma, all of which were Serous Adenocarcinoma. 92.85% cases of ovarian carcinoma expressing PD L1 belong to the high grade category as per FIGO. 92.85% cases of ovarian carcinoma expressing PDL1 belong to the stage three category of TNM, indicating PDL1 expression to be a marker of poor prognosis. Administration of chemotherapy was found to have no effect on the expression of PDL1 in the tumour cells.
ConclusionPD L1 expression was associated with higher grade of ovarian carcinoma and with advance stage of the disease, suggesting it to be a marker of poor prognosis.
