Background <p>Gemcitabine is a nucleoside analogue commonly used in advanced ovarian cancer therapy and, also in various cancers. While it is generally well tolerated, rare adverse effects such as peripheral edema and hypersensitivity may occur.</p> Case presentation and conclusion <p>We report a case of a 54-year-old woman who developed bilateral pitting edema with burning sensation during gemcitabine maintenance therapy. The edema resolved following drug withdrawal and oral dexamethasone. Other causes were ruled out, and a Naranjo score of 7 indicated a probable drug-related event. This case highlights the importance of recognizing gemcitabine-induced edema early to prevent misdiagnosis and ensure timely management without compromising cancer treatment.</p>

Background
