<p>A sole clinical recovery could not always precede a patient’s discharge, especially in the case of homelessness; the&#xa0;treating team faces hurdles due to challenges in individual domains, including poor insight, noncompliance with medication, lack of social skills and functioning, and environmental domains such as absence of or unsupportive family members, inadequate housing facilities, and economic and employment issues. Case management provided by the psychiatric social work team is aims to ease out individuals and their environmental challenges. This case report provides a detailed account of the psychosocial interventions implemented through a case management approach for a patient diagnosed with Bipolar affective disorder and also may assist psychiatric social workers in intervention planning for similar cases.</p>
