The approach to polyuria-polydipsia syndrome has changed substantially in recent years. The new terminology, based on pathophysiology, distinguishes among AVP deficiency, AVP resistance, and primary polydipsia, thus reducing confusion with diabetes mellitus. The diagnostic work-up has also been completely revised following the availability of copeptin measurement. After baseline assessment, hypertonic saline-stimulation test is now considered the gold standard, thanks to its very high diagnostic accuracy. Its use, together with the arginine test, has made the water deprivation test obsolete.
