Zoledronic acid is the most powerful among bisphosphonates with the longest effect on bone. This is mainly caused by a number of factors, including avid uptake by skeletal tissue and greater inhibition of enzyme farnesyl pyrophosphate synthase in the mevalonate pathway, A number of schedule administrations have been suggested, based on its ability to decrease bone resorption for a long period of time. It is the bisphosphonate of choice in avoiding the rebound phenomenon in patients discontinuing denosumab.
