<p>Neuroendocrine tumours (NETs) are often associated with disturbances in glucose metabolism, which may result from therapies, surgical interventions, or hormone hypersecretion by the tumour itself. Additionally, pre-existing diabetes mellitus (DM) is a known risk factor for NETs and can influence their clinical course. The strong correlation between DM and NETs, which share several molecular mechanisms, has sparked interest in the potential antitumor effects of antidiabetic drugs, particularly newer classes such as SGLT2 inhibitors, DPP-4 inhibitors, GLP-1 receptor agonists, and dual GIP/GLP-1 agonists. These agents have shown promising effects on tumour growth, though concerns about their oncological safety remain. This review summarises current evidence on the management of diabetes in patients with NETs, highlighting the importance of personalised therapeutic strategies based on the aetiology of diabetes, coexisting comorbidities, and the patient’s life expectancy.</p>
