<p>Medullary thyroid carcinoma (MTC) is a malignant tumour arising from the parafollicular C cells of the thyroid. It can occur in sporadic or hereditary forms, and surgery is the primary treatment. Ultrasound examination and, in selected cases, anatomical imaging procedures are adopted to stage the disease before primary surgery, while various anatomical/morphological and functional/molecular imaging procedures can be adopted to detect persistent/recurrent disease. Positron-emitting radiopharmaceuticals, including fluorine-18 fluorodeoxyglucose (18F-FDG), fluorine-18 dihydroxyphenylalanine (18F-FDOPA), and gallium-68-labeled somatostatin analogues (68Ga-SSA) have been shown to be useful in detecting MTC relapse/metastasis. This review illustrates the main radiopharmaceuticals for positron imaging of MTC and provides practical indications for rational clinical use.</p>
