Impact of retrieved oocyte number on IVF outcomes in polycystic ovary syndrome: a retrospective cohort study

Do different numbers of oocytes retrieved from women with polycystic ovary syndrome (PCOS) influence treatment outcomes and pregnancy-related complications?
MethodsRetrospective cohort study from The First Affiliated Hospital of Guangxi Medical University and The affiliated hospital of Guilin Medical University. The study population comprised 919 PCOS patients and 4086 women with tubal factor infertility who underwent IVF/ICSI in centers between January 2020 and May 2023. The clinical data of different oocyte retrieved groups were retrospectively analyzed, including baseline characteristics, laboratory parameters, and clinic outcomes.
ResultsThere were significant differences in age, type of infertility, AFC, FSH, T, LH, and AMH among groups PCOS ≤ 10, PCOS > 10, Tubal ≤ 10, and Tubal > 10 (all P < 0.05). In the same oocyte groups, the age of PCOS was younger than tubal factor infertility (PCOS ≤ 10 vs. Tubal ≤ 10, PCOS > 10 vs. Tubal > 10; all P < 0.05). The miscarriage rate was higher in patients with PCOS than tubal infertility (P < 0.05). The number of oocytes retrieved in PCOS patients was a protective factor for the miscarriage rate of fresh transplant cycles considering age or other potential confounders, adjusted odds ratio (OR) 0.926, 95% confidence interval (CI) 0.867 to 0.989 (P < 0.05).
ConclusionsThis study demonstrated that PCOS patients exhibit reduced reproductive potential and higher miscarriage rate compared to tubal factor infertility cases. However, increased oocyte retrieval numbers served as a protective factor against miscarriage in fresh IVF cycles of PCOS patients. These findings underscored the importance of oocyte yield as a prognostic indicator in women with PCOS infertility management during in vitro fertilization.
