<p>Bladder herniation into the inguinal canal is a rare entity, observed in approximately 1–4% of inguinal hernias, and often remains clinically silent. We present the case of a 77-year-old male with a known inguinal hernia and benign prostatic hyperplasia who experienced progressive left flank and groin pain over several years. Serial CT imaging demonstrated gradual herniation of the left bladder wall into the inguinal canal, leading to ureteral compression, hydronephrosis, and ultimately irreversible left renal atrophy. Despite multiple surgical recommendations, the patient had deferred intervention. This case uniquely illustrates the time-dependent evolution of bladder herniation and its potential to cause silent but severe upper urinary tract damage if not promptly recognized and treated.</p>
