Intervention During Normothermic Machine Perfusion of Solid Organs: a New Era in Organ Revitalization

This review aims to summarize the literature and explore the various interventions that have the potential to improve transplant outcomes by harnessing the technology of normothermic machine perfusion in liver and kidney grafts prior to organ transplantation.
Recent FindingsA systematic literature search of PubMed, Scopus, and ClinicalTrials.gov registry was performed. A systematic literature search of the National Library of Medicine database and Scopus identified 1,031 records. Initially, 579 duplicates were removed. Based on the title and abstract, 418 papers were excluded, and 34 articles underwent full-text analysis. Of these, 20 were excluded as they did not meet the study inclusion criteria. Finally, 14 articles were identified that focused on interventions during kidney and liver revitalization with normothermic machine perfusion.
SummaryOur review summarizes the literature regarding the application of normothermic machine perfusion and its potential to serve as a medium for ex-vivo resuscitation and treatment of allografts prior to transplantation. This approach can optimize marginal donor grafts and expand the number of grafts available in the context of the current organ shortage relative to the number of patients on the waitlist. These interventions also have the potential to optimize clinical outcomes for kidney and liver transplant recipients using the current standard of grafts.
