Purpose <p>To evaluate the impact of tumor load on the normal organs mean, maximum standardized uptake values (SUVmean, and SUVmax) of <sup>18</sup> fluorine-labeled prostate-specific membrane antigen (<sup>18</sup>F-PSMA-1007) in positron emission tomography (PET) in patients with prostate cancer.</p> Materials and methods <p>The study involved 175 patients performed <sup>18</sup>F-PSMA-1007 PET/CT. Tumor load was visually classified as high, medium, or low. The SUVmean, and SUVmax were extracted for normal organs and their association with tumor load was determined. Distribution differences and the percentage reductions of normal organ activity across different tumor load groups were estimated.</p> Results <p>Higher tumor load was associated with decrease in SUVmean, and SUVmax of the salivary (<i>P</i> &lt; 0.001, <i>P</i> = 0.005), lacrimal glands (<i>P</i> = 0.003, <i>P</i> = 0.028), blood pool (<i>P</i> &lt; 0.001), spleen (<i>P</i> = 0.044, <i>P</i> = 0.016), kidneys (<i>P</i> &lt; 0.001), and red marrow (<i>P</i> = 0.004, <i>P</i> = 0.007) respectively. No association between tumor load and hepatic SUVmean, and SUVmax (<i>P</i> = 0.217, <i>P</i> = 0.346). SUVmean was reduced by 28.7%, 15.4% and 31% in the salivary, lacrimal glands and kidneys respectively in high tumor load compared to low tumor group. Additionally, SUVmean was reduced by 15% in both lacrimal glands and kidneys in medium tumor load group compared to low tumor load.</p> Conclusion <p>The biodistribution of PSMA in healthy tissue is influenced by tumor burden, suggesting that higher dosages of radioligand therapy can be given before adverse events on critical organs become relevant.</p>
