Adolescents and young adults with chronic childhood conditions (AYACCC) transitioning from pediatric to adult-centered healthcare models encounter challenges including identifying adult healthcare providers equipped to provide their complex medical care. This review aimed to examine effective adult healthcare models for AYACCC.
Recent FindingsThe heterogenous nature of individual’s disease processes, needs, and community structures necessitate varying models of healthcare. These can include subspecialty-driven clinics, disease-specific clinics, consultative models, and medical home models. Each of these offers unique advantages and can be sought within a provider’s area to identify or build partnerships and care networks.
SummaryAYACCC transitioning to adult healthcare require a flexible, individualized approach, with different care models offering distinct advantages.
