Purpose of Review <p>Postoperative complications adversely affect outcomes and result in shorter disease free and overall survival. As the population demographic becomes older and frailer, the demand for effective postoperative care is escalating. Currently, there is a notable disparity between postoperative ward-based care and the care provided in high dependency units (HDU) and intensive care units (ICU). This gap exposes intermediate-risk patients, with limited access to HDU/ICU facilities, to increased risk of postoperative morbidity and mortality and has significant health economic implications. Strategies for reducing early postoperative morbidity and mortality following surgery must focus on implementing policies which enhance perioperative care systems.</p> Recent Findings <p>Mounting evidence supports preventative approaches with the use of specialized, anaesthesia-led postoperative care through enhanced recovery room management to bridge this care gap effectively. This review advocates for the development of enhanced recovery room management as routine parts of hospital care for intermediate risk, and particularly surgical oncological patients.</p> Summary <p>Anaesthetists, have a critical role in delivering on the value proposition of perioperative care and are ideally positioned to lead this transformative approach. This review summarises the characteristics of Enhanced Care Units (ECUs), the current evidence for patient and health economic benefits from these units, offer insights into how best to implement these models into existing hospital systems and proposes avenues for future trials investigating which patients will benefit most from enhanced post-operative care models.</p>
