Current Recommendations and Special Considerations in the Care of Patients with Burns

The purpose of this review is to examine the current literature published on nutritional recommendations in patients who suffer burn injuries. We aimed to evaluate available data, incorporate new findings, and synthesize current recommendations into a comprehensive, yet easy-to-reference review.
Recent FindingsOngoing support for early, within 24 h, initiation of enteral nutrition. Routine glutamine supplementation is not recommended. Similarly, high-dose vitamin C supplementation is not recommended. A growing body of evidence is noted for safety of post-pyloric feeding, including intraoperatively.
SummaryEarly enteral nutrition (EN) is fundamental. EN has a multitude of benefits including dampening the hypermetabolic state, protects against muscle wasting, and reverses immune suppression. Indirect calorimetry remains the gold standard for determining caloric needs. Burn patients require a protein-rich diet. Post-pyloric feeds provide a lower aspiration risk and decrease interruptions in nutrition. Adjuncts such as oxandrolone and propranolol provide a benefit to this population.
