Auswirkungen durch Advanced Practice Nursing in der Versorgung von Menschen mit kardiovaskulären Erkrankungen – ein systematisiertes Review

Cardiovascular diseases represent a major challenge in the care of chronically ill patients. Advanced Practice Nurses (APNs), nursing professionals with advanced knowledge and specialized clinical skills, are increasingly taking on complex care responsibilities in this context; however, the current body of evidence regarding their specific impact on the health outcomes of this population remains limited and inconsistent.
ObjectiveTo summarize the existing knowledge on the possible effects of APN care on the health status of people with cardiovascular diseases and to identify further research needs.
MethodA systematized review was conducted following the typology described by Grant and Booth. The literature search was carried out in two scientific databases and supplemented by manual searching. Included were studies investigating the effects of APNs on the health outcomes of patients with cardiovascular diseases. Inclusion criteria comprised empirical studies using qualitative, quantitative or mixed methods designs published between 2014 and 2023. The data were analyzed using an inductive qualitative content analysis according to Mayring, complemented by critical appraisal based on CASP checklists (Critical Appraisal Skills Programme) and criteria-guided analysis following Mayer et al.
ResultsA total of 17 studies were included in the analysis. The qualitative content analysis identified three key themes: (1) physical and psychological health status, (2) illness perception, self-care and risk management and (3) health literacy, lifestyle and quality of life. On the individual level, APNs demonstrated positive effects in promoting physical activity, supporting disease coping, improving adherence and enhancing emotional stability. In addition, they contributed to the reduction of cardiovascular risk factors. Digital and gender-sensitive interventions proved to be particularly effective. Several studies also highlighted structural barriers to the use of rehabilitation services, underlining the importance of low-threshold socially contextualized nursing interventions.
ConclusionThe findings highlight that APNs make a significant contribution to improving the health outcomes of patients with cardiovascular diseases. Their impact unfolds across multiple levels. particularly through educational, motivational, and coordinating interventions. To fully leverage the potential of APNs, their competencies should be systematically integrated into care structures and strengthened through supportive health and education policies. Future research should focus on long-term effects, social determinants and the effectiveness of digital and participatory nursing interventions.
