<p>This study aimed to assess the analgesic and anti-inflammatory effects of the aqueous extract of <i>Maesa lanceolata</i> leaves (AEML) on gouty arthritis induced in rats by the intra-articular injection of 5&#xa0;mg of monosodium urate (MSU) into the left ankle. Following MSU injection, rats were treated for six days with allopurinol (5&#xa0;mg/kg/day) or AEML at 45, 90, or 180&#xa0;mg/kg/day. Ankle and paw diameters, as well as pain sensitivity, were measured daily. After treatment, brain and spinal cord tissues were collected to measure nitric oxide (NO), malondialdehyde (MDA), glutathione (GSH), and superoxide dismutase (SOD) levels. Cytokine levels (IL-1β and TNF-α) were analyzed in the serum and synovial fluid. Histological analysis of the ankle was conducted, and in vitro studies were performed to assess the anti-inflammatory activity of AEML on macrophages under lipopolysaccharide (LPS) or MSU stimulation. AEML significantly reversed MSU-induced hyperalgesia and reduced ankle and paw swelling. It decreased NO and MDA levels and increased SOD activity in the spinal cord. AEML significantly increased GSH levels in the left-brain hemisphere. Histological analysis revealed reduced inflammation in ankle tissues from AEML-treated animals. However, AEML did not significantly reduce serum or synovial cytokine levels. In vitro, AEML was noncytotoxic and reduced NO production but unexpectedly increased cytokine levels under both normal and inflammatory conditions. AEML alleviated gout-induced pain and inflammation, with effects unrelated to cytokine reduction. These findings suggest alternative anti-inflammatory mechanisms, highlighting AEML as potential therapeutic agent for gouty arthritis.</p>
