Background <p>Vitamin D deficiency has been identified as a contributing factor for diabetes mellitus (DM) through its potential role in compromising innate immunity. Diabetic foot ulcer (DFU) patients are at a higher risk of infections and delayed wound healing.</p> Objective <p>This systematic review aims to evaluate whether there is a significant link between vitamin D status and the occurrence of diabetic foot ulcers.</p> Methods <p>A comprehensive literature search was conducted in PubMed, Embase, Google Scholar, and the Cochrane Library databases up to May 30, 2023. Sixteen studies that met the inclusion criteria were included in the analysis. The quality of the studies was assessed using the Newcastle–Ottawa Scale. A total of 8341 patients were analyzed, including 2019 DFU patients and 6322 diabetic patients without foot ulcers. Meta-analysis was performed using the R software environment (version 4.2.2).</p> Results <p>The meta-analysis demonstrated a significant association between 25(OH)D levels and the occurrence of DFU. The weighted mean difference (WMD) in the random effects model was −4.13 ng/mL (95% CI −5.04 to −3.22, I² = 64%, <i>p </i>&lt; 0.01).</p> Conclusion <p>There is a significant association between vitamin D deficiency and the presence of DFU in diabetic patients. Further studies are required to determine the efficacy of vitamin D supplementation in improving clinical outcomes for DFU patients.</p>
