A Retrospective Cohort Study Comparing Barbed Versus Non-Barbed Sutures For Vaginal Vault Closure Following Minimally Invasive Hysterectomy

Vaginal vault closure is a crucial step in minimally invasive hysterectomy, and the choice of suture material can influence operative efficiency and postoperative outcomes. Barbed sutures, with their knotless self-anchoring design, may overcome technical limitations of conventional polyglactin sutures. This study compares barbed and non-barbed sutures for vault closure with respect to operative time, postoperative complications, and suture absorption.
Materials and MethodsThis retrospective cohort study included 204 women who underwent laparoscopic or robotic hysterectomy for benign gynecological indications between July’2016 – April’2024. Vaginal vault closure was performed using either vicryl-1 (non-barbed; n=100) or V-Loc 1-0 (barbed; n=104). Data on demographic variables, comorbidities, suturing time, postoperative complications, and suture absorption were collected. Follow-up was at 48 hours, 1 week, and at 6, 10, and 12 weeks. Statistical analysis used Chi-square and Student’s t-tests; p<0.05 was considered significant.
ResultsBaseline demographic and clinical characteristics were comparable between groups. Mean suturing time was significantly shorter with barbed sutures (10.28 ± 1.94 vs 12.69 ± 3.31 minutes; p < 0.0001). Secondary hemorrhage (2.88% vs 12%; p = 0.0126) and vault granuloma (1.92% vs 10%; p = 0.0142) were significantly lower with barbed sutures. One case of vaginal cuff dehiscence occurred in the non-barbed group. Suture absorption was delayed with barbed sutures, with only 43.26% absorbed by 12 weeks compared to complete absorption of vicryl by 10 weeks.
ConclusionBarbed sutures offer improved operative efficiency and lower postoperative complication rates for vaginal vault closure in minimally invasive hysterectomy. However, delayed suture absorption warrants appropriate patient counseling.
