Comparison of Dedicated Inserter Versus Kelly’s Forceps for Postpartum Intrauterine Contraceptive Device Insertion: Randomized Controlled Trial

Traditionally, Kelly’s forceps is used for postpartum intrauterine contraceptive device (PPIUCD) insertion, but dedicated inserter has several advantages. The aim of this study was to compare the use of dedicated inserter with conventional Kelly’s forceps for PPIUCD insertion.
Study designThe study was a prospective, interventional, single-blinded, randomized controlled trial conducted between January 2022 and November 2023. Patients were randomized into: PPIUCD insertion using dedicated inserter (Group 1) and using Kelly’s forceps (Group 2). Patients were followed up at 6 weeks and 3 months postpartum. Fundal placement of IUCD, provider and participant’s satisfaction rate, and any complications encountered were assessed.
ResultsHundred patients were recruited. Group 1 had significantly higher fundal placement of IUCD. (p = 0.02) Providers were more satisfied in group 1 (78% Vs 52% in group 2). (p = 0.001) The dedicated inserter was found to require lesser manipulation and shorter training period and achieved higher fundal placement. Participants experienced less pain in group 1 (median VAS = 2 Vs 5 in group 2). At 6 weeks and 3 months sonography, the IUCD position was comparable in both the groups. Spontaneous expulsion was 14% and 8% and partial expulsion 2% and 8% in group 1 and group 2, respectively. At 3 months follow-up, there were no additional expulsions seen in either group.
ConclusionThis dedicated PPIUCD inserter performed as intended and was found to be safe, with high acceptability among the participants and providers. The dedicated inserter has better ease of insertion and high fundal placement as compared to the conventional technique.
