Introduction <p>Analysis of Maternal Near-Miss cases is an alternative to study of maternal deaths. Maternal Near-miss and Maternal mortality are the indicators of the health system serving pregnant women. This helps in formulating and aiding the strategies which help in preventing maternal deaths.</p> Aim <p>To study the characteristics of the Maternal Near-Miss cases at a tertiary care institute.</p> Materials and Methods <p>This is a Retrospective study of the Maternal Near-Miss cases admitted in the Intensive Care Unit of Cheluva Mba Hospital - Department of Obstetrics and Gynecology, Mysore Medical College and Research Institute (MMCRI), Mysore, Karnataka, India, for a period of 5 years, from January 2019 to December2023. Patient characteristics like age, parity, gestational age and other risk factors were analyzed.</p> Results <p>There were a total of 47,148 deliveries during the study period, out of which 585 were Maternal Near-Miss cases and 100 maternal deaths. The Maternal Near-Miss ratio was 12.4/1000 live births. Maternal Near-Miss to Mortality ratio was 5.8:1 and Mortality Index was 14.5%. Hypertensive disorders of pregnancy was the leading cause of Maternal Near-Miss.</p> Conclusion <p>There is shift in the paradigm of cause of maternal mortality and near-miss. Currently, globally in developed countries and in developing countries with good maternal care, hypertensive disorders of pregnancy have emerged as a leading cause of maternal mortality, surpassing obstetric hemorrhage. This study highlights the need to establish High Dependency Units in maternal and child health care, implement multidisciplinary management, and improve the organization of referral units.</p>
