Introduction <p>As India is the most populous country in the world, the use of safe and effective contraception is the need of hour. Despite governments’ diligent efforts, the acceptance of long-acting reversible contraceptive methods (LARCs) like copper intrauterine device and depot medroxyprogesterone acetate (DMPA) is very low.</p> Aim <p>To compare the contraceptive efficacy of both methods, their continuation rate, failure rate, and side effects among users.</p> Methodology <p>This prospective descriptive study was conducted in the department of obstetrics and gynaecology, government medical college Kota, Rajasthan, on 143 women over 18 months from December 2022 to May 2024. Seventy women opted LARCs as interval, while 73 as post-abortion contraceptive.</p> Results <p>The mean age of women was 28.19 years; the most common complaint in IUD users was heavy menstrual bleeding and in DMPA users was decreased flow during periods. The acceptance of Cu-T during follow-up visits declined to 96.20 % at 6 weeks, 67.08% at 3 months, and 40.50 % at 6 months, whereas in DMPA users at 6 weeks it was 100 %, 56.25% at 3 months, and 43.75% at 6 months.</p> Conclusion <p>Cu-T 380-A is more accepted as post-abortion contraceptive, whereas DMPA as interval contraceptive. Continuation of DMPA was more than Cu-T 380-A. The most common complaint was heavy menstrual bleeding in Cu-T users and decreased flow during periods followed by intermenstrual spotting in DMPA. The efficacy of DMPA was found more than of Cu-T 380-A.</p>
