A Single Institution Experience with Pancreatic Carcinoma: A Retrospective Study

This paper aims to showcase the trends of pancreatic cancer treatment based on the data obtained from a single high-volume center in western India. This study is a retrospective observational study of all the patients who presented to the outpatient department of a tertiary cancer care center in Gujarat from 2018 to 2023. The analysis is done on Jamovi version 2.6.22 software and includes descriptive statistics, univariate analysis, and survival analysis. The results of treatment at our institute have shown similar outcomes to trends worldwide. Use of operative techniques such as Whipple + TMpE (N = 48) has resulted in better lymph node yield without increased rates of complications as compared to Whipple procedure (N = 39) Increased lymph node yield (> 12 LN) allowed for identification of poor prognostic factors such as perinodal spread, LVI, and PNI. Only 87 were able to undergo surgery, out of which 47 were male. The mean age was 54.4 years. Surgeries performed for these patients and their overall survival (OS) at 2 years is as follows: PRPD (34.1%), PPPD (33.4%), distal pancreatico-splenectomy (0%), RAMPS (50%). The majority of the patients had adenocarcinoma of pancreas (81.6%)with the other histopathologies being NET (17.3%) and SPEN (1.1%). This study highlights the trends of this disease at a tertiary cancer care institute in India. The use of meticulous technique for surgery with improved postoperative monitoring has resulted in better outcomes. Further improvement in results can be achieved with use of appropriate neoadjuvant and adjuvant chemotherapy, which is still a matter of research.
