Introduction&#xa0; <p>Genital self-image plays a significant role in women’s sexual well-being and relational satisfaction, yet little is known about its impact in the postmenopausal period. This study aimed to investigate the relationship between genital self-image and sexual quality of life, marital adjustment, and life satisfaction among postmenopausal Turkish women.</p> Methods <p>This cross-sectional study was conducted with 268 postmenopausal women attending outpatient clinics at a State Hospital in Türkiye. Participants completed the Female Genital Self-Image Scale, the Sexual Quality of Life-Female Scale, the Marital Adjustment Scale, and the Life Satisfaction Scale. Data were analyzed using descriptive statistics, Pearson correlation, and hierarchical linear regression analyses.</p> Results <p>The mean score for the Female Genital Self-Image Scale was 17.24 ± 3.34, within the possible range of 7–28, indicating a moderate level of genital self-image among participants. The findings showed a moderate positive correlation between genital self-image and sexual quality of life (<i>r</i> = 0.512, <i>p</i> &lt; 0.001), marital adjustment (<i>r</i> = 0.480, <i>p</i> &lt; 0.001), and life satisfaction (<i>r</i> = 0.422, <i>p</i> &lt; 0.001). In the hierarchical regression analysis, demographic and clinical characteristics were entered in the first step, and genital self-image was added in the second step. Genital self-image accounted for 31% of the variance in sexual quality of life, 23% in marital adjustment, and 24% in life satisfaction.</p> Conclusions <p>Postmenopausal women with a more positive genital self-image demonstrated significant associations with higher sexual quality of life, marital adjustment, and overall life satisfaction.</p> Policy Implications <p>These results highlight the need for multifaceted education and support programs during the postmenopausal period. In addition to health professionals, other stakeholders—including media, educators, and community organizations—can play a crucial role in promoting sexual quality of life, strengthening marital adjustment, and enhancing overall life satisfaction among postmenopausal women.</p>
