<p>This article aims to search the literature for innovations in telemedicine, by a bibliometric study, to characterize the state of the art and identify research trends in the literature. A total of 154 articles related to innovations in telemedicine, published between 2017 and January 2021, were selected from the Web of Science database using the keywords "innovation" and "telemedicine." These articles were analyzed to track the evolution of the literature, identify frequent research areas, the most cited articles, the journals with the highest publication frequency, co-citation maps of authors, the main countries publishing on innovations in telemedicine, and research trends. The analysis employed VOSviewer software for co-citation mapping, IBM SPSS and Excel for quantitative data analysis, and Iramuteq software for qualitative analysis through Similitude Analysis, which identified lexical relationships. Only peer-reviewed journal articles were included, with conference proceedings and books excluded from the selection criteria. The research trends in the literature are: a) innovative business models aimed at telemedicine; b) impacts, quality, user acceptance, and obstacles to digitization in medicine; c) management, operation, and costs of digitalization in medicine; d) imperative of digitalization in medicine in the COVID-19 scenario. In addition, there was an increase in publications on this field of study in 2019 and 2020, mostly related to COVID-19. The most frequent research area is “Health Care Sciences &amp; Services” (31.2%), with 13% of these studies being interdisciplinary with the area of “medical informatics”. This research contributes to characterize an emerging field of study and its research trends, which serves as a guide for understanding the field and enables researchers to direct their future research to little or unexplored areas. It also emphasizes that innovations in telemedicine contribute to sustainability by promoting long-term efficiency, reducing environmental impacts, and fostering equitable access, aligning with SDG 3 and SDG 9. There is no published article in the field of study that conducts a bibliometric analysis of innovations in telemedicine to understand how these innovations are being investigated without confining them to a specific knowledge domain, whether it is healthcare, management, or information systems.</p>
