Kardiologie trifft Rheumatologie

This paper is based on the lecture “Cardiology Meets Rheumatology” given at the 35th annual meeting of German Society for Paediatric Rheumatology (GKJR) in Garmisch-Partenkirchen. The main overlapping topics will be introduced. One of them is the nowadays acknowledged increased risk of atherosclerotic heart disease in rheumatic diseases. We will however focus on cardiac complications and their prevalence among children with inflammatory connective tissue disorders and on Kawasaki disease with the pitfalls of coronary artery imaging. The cooperation between the two specialties will be illustrated using three case reports. These will also demonstrate new developments at the interface between rheumatology and cardiology. Specifically, the use of biologics in the treatment of inflammatory cardiac conditions and the use of cardiac magnetic resonance imaging (MRI) to detect myocardial and pericardial inflammation. In the first case, the diagnostic and therapeutic approach to idiopathic recurrent pericarditis will be discussed. The second case describes perimyocarditis as a complication of juvenile dermatomyositis manifestation. Potentially relevant consequences, such as rhythm disorders and the development of cardiomyopathy as a result of a subclinical perimyocarditis will be mentioned here. At the same time, the role of cardiac MRI will become evident. The last case will illustrate pitfalls associated with echocardiographic imaging of coronary arteries. Open and direct communication between cardiologists and rheumatologists will be emphasized in all cases.
