<p>Primary pulmonary choriocarcinoma (PPC) in males is an extremely rare extragonadal malignant trophoblastic tumor with an unclear pathogenesis. Its clinical manifestations are heterogeneous and often mimic more common conditions, leading to frequent misdiagnosis. Moreover, PPC is characterized by early metastatic potential, and currently, no standardized treatment guidelines exist. Here, we report the case of a male patient with PPC who was treated with a combination of chemotherapy and immunotherapy. We also provide a comprehensive review of the literature to enhance understanding of this rare entity, improve early diagnostic accuracy, and explore potential therapeutic strategies.</p>
