Background <p>OSMF is a chronic, progressive, potentially malignant disorder affecting the oral cavity and, occasionally, the pharynx. The present study aimed to evaluate the impact of chewing habits on the clinical and functional severity of OSMF.</p> Materials and Methods <p>A cross-sectional study was conducted among 343 OSMF patients. Clinical and functional staging were assessed using a disease progression–based classification. Demographic details and habit-related characteristics were recorded. Statistical analysis was performed using the Chi-square test, Spearman’s correlation coefficient and Multivariate ordinal logistic regression analysis.</p> Results <p>Clinical stage 3 (S3) and functional stage 2 (M2) were the predominant stages (46.9% and 50.1%, respectively). Significant associations were observed between age, habit frequency, habit duration, and type with both clinical and functional staging of OSMF (<i>p</i> &lt; 0.05). Functional staging demonstrated a significant association with clinical staging (<i>p</i> &lt; 0.001). Clinical staging showed a significant positive correlation with functional staging (<i>r</i> = 0.490, <i>p</i> &lt; 0.001), habit frequency (<i>r</i> = 0.184, <i>p</i> &lt; 0.001), and habit duration (<i>r</i> = 0.111, <i>p</i> = 0.040). Multivariable ordinal logistic regression analysis revealed only age as a significant predictor of higher clinical and functional stage severity, while other habit-related variables showed no statistically significant associations.</p> Conclusion <p>While univariate analysis demonstrated significant associations between chewing-habit variables and disease severity, the multivariable regression analysis identified age as the only independent predictor of increasing severity. The significant association between clinical and functional staging in OSMF indicates that both are distinct but complementary indicators of OSMF Severity.</p>
