Sagittal Split Osteotomy of the Mandibular Ramus for Removal of “Kissing” Molars: A Case Report

The “Kissing molars” represent a rare example of dental impaction. This phenomenon occurs when the occlusal surfaces of impacted molars come into contact within the same follicular space. The surgical procedure may present an increased risk of injury to the inferior alveolar nerve and a higher possibility of mandibular fracture during or after surgery. The sagittal split osteotomy of the mandibular ramus, when applied for tooth extraction purposes, allows the surgeon to reduce the risk of inferior alveolar nerve injury and other major complications.
Case PresentationA clinical case of kissing molars with severe impaction, in which the extraction of the second and third molars was performed using the sagittal split osteotomy of the mandibular ramus. As demonstrated in the present case, the technique provided excellent access and visualization of the entire surgical area, allowing direct visualization of the inferior alveolar nerve, with no intraoperative or postoperative complications and low morbidity.
ConclusionsThe Sagittal Split Osteotomy of the Mandibular Ramus is an effective technique for the removal of deeply impacted third molars. It serves as an alternative to conventional dental extraction.
