Seed and Soil Theory Revisited: Donor Site Recurrence in Head and Neck Cancer with a Case Report and Literature Review

Restoring the aesthetic and functional normalcy of the head and neck region post resection for malignant growths is a daunting challenge for the reconstructive surgeon. Recurrence of the primary disease at the flap donor site is a rare complication with potentially serious but avoidable sequelae. These complications can be prevented by certain routine practices like change of gloves and instruments before harvesting the flap for reconstruction. In this article we review previous reports of such complications described in literature and also report a possible case of an implantation recurrence of a carcinoma of the buccal mucosa at the Pectorali Major Myo-cutaneous (PMMC) flap donor site.
Case ReportsA 54-year-old lady after undergoing a composite resection for squamous cell carcinoma of the left gingivobuccal sulcus (GBS) with reconstruction with a PMMC flap, developed an implantation recurrence in the donor site of the flap 18 months after the primary surgery. The patient subsequently underwent palliative chemotherapy followed by left mastectomy with removal of the axillary mass. The treatment was consolidated with adjuvant radiation.
ConclusionImplantation recurrence is a rare but theoretically plausible complication of reconstructive surgeries for oral cavity malignancies. Although causation remains speculative, basic precautions like change of gloves, instruments and surgical drapes will go a long way in preventing such complications.
