Retrieval of a Third Molar Displaced into the Pterygomaxillary Fossa in a Pediatric Patient Using CT-Based Three-Dimensional Localization for Surgical Planning: Case Report and Literature Review

Displacement of maxillary third molars into deep anatomical spaces is a rare complication. A 13-year-old patient underwent third molar extraction, during which tooth 18 was displaced into the pterygomaxillary fossa. CT-based three-dimensional localization enabled precise identification of the displaced tooth and guided surgical decision-making. A delayed intraoral retrieval approach was performed after three weeks, allowing stabilization of the tooth and reducing the risk of further migration. No complications were observed after 18 months. This case highlights the clinical value of CT-based three-dimensional localization in improving surgical safety and outcomes in anatomically complex scenarios.
