Introduction <p>Zygomatic complex fractures commonly produce pronounced periorbital edema and ecchymosis that may persist despite routine measures such as cold compresses, delaying functional and aesthetic recovery. Hyaluronidase degrades extracellular hyaluronic acid, reducing tissue viscosity and potentially enhancing resorption of interstitial fluid. This pilot randomized trial evaluated whether periorbital hyaluronidase accelerates resolution of edema and ecchymosis after zygomatic complex fracture repair.</p> Material and Method <p>This randomized clinical trial enrolled 24 adults (18–70 years) with unilateral or bilateral zygomatic complex fractures planned to be treated with open reduction and internal fixation. Participants were randomized (1:1) to cold compresses twice daily (Group A, <i>n</i> = 12) or periorbital infiltration of 750 IU hyaluronidase divided across three standardized injection points (Group B, <i>n</i> = 12). Standardized photographs were obtained at baseline and 24-hour intervals up to 168&#xa0;h. A blinded evaluator assessed edema and ecchymosis using SPREE scale.</p> Results <p>All participants completed follow-up with no adverse events observed. Both groups improved over time (<i>p</i> &lt; 0.001). Hyaluronidase produced earlier and greater improvement, with significant between-group differences from day 3 for edema and day 2 for ecchymosis; near-complete edema resolution occurred in 83.3% by day 7 versus none in controls, and complete ecchymosis clearance 75% of hyaluronidase group versus none in control.</p> Conclusion <p>Periorbital hyaluronidase infiltration appears safe and significantly accelerates resolution of post-traumatic periorbital edema and ecchymosis after zygomatic complex fracture. Larger, multicentre randomized trials are required to validate these findings and refine dosing, timing, and injection-site protocols for routine clinical use.</p>
