Background <p>The purpose of this study was to evaluate the efficacy of active phasecallous molding(CM) in solving many problems associated with distractionosteogenesis (DO) of mandible like anterior open bite(AOB), reverse overjet,transverse discrepancy, worsening of facial asymmetry and malocclusion in 30patients of temporomandibular joint ankylosis aged from 4–42 years.</p> Material and method <p>Patients undergoing unilateral or bilateral, uniplanar/biplanar orcombination of DO, either before, after, or concurrently with ankylosis release wereevaluated after CM for AOB correction, posterior open bite creation, superior-inferiordifferential distraction, Scissor/Crossbite correction, midline correction, functionalocclusion, loss of distraction &amp; relapse. Clinical data were recorded and analyzed.Result: The mean length of corpal distraction was 24.63 mm &amp; ramal distraction was15.50mm. AOB correction to normal was achieved in 85%. Average POB of 7 mmwas achieved in all ramal distraction cases. A mean of 5mm superior-inferiordifferential distraction was achieved. Correction of scissor bite was achieved in 82%and Crossbite correction was achieved in 90% of patients. Midline correction to 0mm was achieved in 66.66%. Functional occlusion was achieved in 75 % of patients.Mean loss of corpal distraction &amp; ramal distraction was 5.5 mm and 2.62 mm, andrelapse was 2.39 mm &amp; 1.82 mm respectively. Complications like hour glass shapecallous and fracture of callous were recorded in 2 cases.</p> Conclusion <p>CM can create wonders in terms of giving movements to the extent anddirections impossible by routine orthodontics, further surgical correction may not berequired, giving the best possible aesthetic and functional outcomes.</p>
