Clinical and Radiographic Outcomes after Filling Peri-Implant Gap with Simvastatin, Alloplast and Platelet Rich Fibrin Around Immediate Implants in Esthetic Zone- A Randomized Clinical Trial

To evaluate the clinical and radiographic outcomes after filling peri-implant gap (PIG) with Simvastatin (SMV), Alloplast or Platelet Rich Fibrin (A-PRF) around single immediate implants in esthetic zone.
Materials and methods37 patients requiring anterior tooth extraction and IIP participated in the trial. PIG was filled with A-PRF, 1.2% SMV gel and alloplast in Group 1, Group 2 and Group 3 respectively. Labial bone thickness (LBT), marginal bone levels, and implant stability were measured at different time intervals till one year after implant placement. Pink Esthetic scores (PES) were assessed before extraction (PES-T0), immediately after delivery of provisional crown (PES-T1), and one month after delivery of definitive crown (PES-T2) along with the evaluation of patient satisfaction.
ResultsMean age of participants was 36.73 years. LBT decreased after 1 year, but the difference was non-significant in all groups (T0-T3, p > 0.05). More changes were evident at 3 mm distance from implant shoulder than those observed at 6 mm. Intergroup differences in marginal bone levels, mean PES and implant stability and patient satisfaction were non-significant at all-time intervals (p > 0.05). Mean ISQ values after one year were 73.09 ± 4.45, 71.90 ± 6.22 and 74.08 ± 6.43 in group 1, 2 and 3 respectively. Within each group, PES scores improved with time and were within clinically acceptable limits (Staas in Clin Implant Dent Relat Res 24(1):24-33;2022), (Bernal in J Prosthet Dent. 2023).
ConclusionClinical and esthetic outcomes were similar in all groups with time, suggesting a novel therapeutic option of Simvastatin gel as PIG filling material. Patient satisfaction was not influenced by the esthetic outcome achieved due to difference in peri-implant graft material.
