This case report presents the management of an odontogenic infection in a 19-year-old female with Takayasu arteritis (TA). The patient had been diagnosed with TA 8 years earlier. She presented with severe pain, swelling, and pus discharge from the right cheek for 15 days. Clinical examination revealed a draining sinus tract associated with an impacted mandibular third molar (tooth 48). Surgical extraction of the impacted tooth and excision of the sinus tract were performed under appropriate antibiotic coverage and medical supervision. The procedure was uneventful and follow-up showed satisfactory healing. This case highlights the importance of careful planning and timely treatment of oral infections in patients with TA, a rare chronic inflammatory disease affecting large vessels. Maintaining oral hygiene and regular dental visits are critical to preventing complications in medically compromised individuals.
