摘要

Background

Superficial temporal artery (STA) pseudoaneurysm is a rare vascular lesion, usually following blunt trauma. Misdiagnosis and inappropriate intervention can precipitate life-threatening hemorrhage.

Case Description

A 30-year-old male kabaddi player presented with active pulsatile bleeding from a swelling over the left temporal region following aspiration at a peripheral center. He had a 3-month history of progressive swelling after repeated head trauma. Examination revealed a 4 × 4 cm compressible pulsatile mass. CT brain showed an extracranial lesion without intracranial extension, consistent with STA pseudoaneurysm. Emergency surgical excision with proximal and distal ligation of the STA achieved hemostasis and definitive cure.

Conclusion

STA pseudoaneurysm should be suspected in post-traumatic temporal swellings. Radiological confirmation with Doppler or CT angiography is essential before any intervention. Aspiration should be avoided. Prompt surgical ligation and excision remain the treatment of choice to prevent catastrophic bleeding.
