<p>Aneurysmal bone cysts (ABC) are benign, non-neoplastic cystic lesions that consist of multiple cavities filled with blood and divided by septa made of connective tissue. They account for 1.5% and 6% of all lesions in the jaw and skull respectively. It is distinguished by a rapid growth pattern leading to bony expansion, erosion and facial asymmetry. This is a case of large ABC in a 25&#xa0;year old male patient affecting the left body of the mandible with ballooning expansion and thinning of the buccal and lingual cortical plates. Aggressiveness and erosion of thick mandibular inferior border mandated for surgical resection with segmental mandibulectomy and reconstruction of defect with free fibula flap. One year follow up showed no evidence of recurrence and maintenance of facial symmetry.</p>
