Background <p>Maxillofacial war injuries in the last war against terrorist groups in Iraq are challenging to manage due to the complicated mechanism of these injuries with the severe disruption of the hard and soft tissue.</p> Objectives <p>This study aims to review patterns and management of war injuries affecting the maxillofacial region during recent armed conflicts against ISIS in Iraq and to investigate the complications resulting from these injuries.</p> Materials and Methods <p>The archive for patients who sustained maxillofacial missile injuries over 3&#xa0;years (2014–2017) was reviewed retrospectively. All these patients have been treated (by the 1st author) at the Maxillofacial Surgery Unit of Al-Shaheed Gazi Al-Hariri Teaching Hospital in the Medical City of Baghdad. All patients are examined clinically and radiographically (plain radiographs or CT scans), and the patients were evaluated and prepared for intervention when indicated based on clinical and radiographical findings.</p> Results <p>A total of 64 patients with 191 fractures, 56.25% of them caused by shell fragments and the rest by bullets, were enrolled in this study and have been followed up for periods ranging from 6&#xa0;months to 2&#xa0;years. The differential site-wise distribution of the mid-face fractures indicated that orbital fractures (orbit's floor and walls) were the highest injuries (32.6%), followed by maxillary sinus wall fractures (22.8%). In contrast, mandibular fractures revealed that the body and the parasymphyseal regions were the most frequently affected (18.2% for each).</p> Conclusions <p>The appropriate management of maxillofacial war injuries requires surgical expertise, considering patient-specific conditions to balance surgical invasiveness and obtaining optimal functional and esthetic outcomes.</p>
