Introduction <p>Osteomas are the most common benign tumors of the paranasal sinuses, predominantly affecting the frontal sinus. While typically slow-growing and asymptomatic, giant osteomas—defined as those exceeding 30 mm—can present with symptoms due to mass effect on adjacent structures. Their occurrence in pediatric patients, especially females, is exceedingly rare.</p> Materials and Methods <p>A 13-year-old female presented with a 4–5-month history of progressive swelling in the left supraorbital region and one month of intermittent blurring of vision in the left eye. Clinical examination revealed a firm, bony-hard swelling deforming the supraorbital rim and displacing the globe. Imaging studies, including non-contrast computed tomography (NCCT) and magnetic resonance imaging (MRI), identified a large, lobulated lesion of the left frontal sinus with orbital and intracranial extension. Surgical excision was performed via a coronal approach, followed by cranialization of the frontal sinus using a pedicled pericranial flap.</p> Results <p>The excised lesion measured 24 × 44 × 33 mm and weighed 42.36 g. Histopathological examination confirmed a mixed-type osteoma composed of compact mature lamellar bone interspersed with fibrous stroma. Postoperatively, the patient experienced complete resolution of symptoms, with no recurrence observed at 6-month follow-up.</p> Discussion <p>Giant frontal sinus osteomas with orbital and intracranial extension are rare in the pediatric population. Early diagnosis through imaging is crucial for timely intervention. Surgical management via a coronal approach provides optimal exposure for complete excision and safe management of sinus and dural integrity. Histopathological confirmation and long-term monitoring are essential for favorable outcomes.</p>
