<p>The sialoliths usually measure ≤ 10&#xa0;mm and commonly involve the submandibular gland or its duct. A few of these calculi enlarge in size to ≥ 15&#xa0;mm and these are then called ‘Giant Sialoliths’. It is extremely rare for sialoliths to grow more than 5&#xa0;cm in the largest dimension and cause complications. To the best of our knowledge, only 25 cases of such giant sialoliths (&gt; 5&#xa0;cm) have been described in the literature. We present here a case of a patient who possibly ignored his sialolith for years due to psychiatric problems and presented to the emergency with a giant submandibular sialolith (&gt; 5&#xa0;cm) with severe airway compromise, due to associated abscess, requiring intubation. We have also provided an update on the literature on giant submandibular sialoliths measuring ≥ 50&#xa0;mm.</p>
