Purpose of Review <p>This manuscript offers an up-to-date review of effective oncologic strategies to extend the eligibility for breast conserving therapy in patients with breast cancer.</p> Recent Findings <p>There is a growing body of literature that supports breast conservation as a safe and effective option in populations previously considered ineligible: patients with recurrent ipsilateral breast cancer, multifocal/multicentric breast cancer, and pregnancy. Appropriate patient selection and multidisciplinary conversations are key principles to increase both oncologic and cosmetic success. Oncoplastic surgical techniques and neoadjuvant therapies have also emerged as effective methodologies to facilitate breast conservation in patients with large tumor size who historically may have required mastectomy.</p> Summary <p>Breast conserving therapy results in decreased morbidity and improved quality of life in comparison to mastectomy. Extending indications for breast conservation, therefore, carries great potential benefit for patients. Providers should be aware of emerging indications and treatment strategies to increase eligibility (Table <InternalRef RefID="Tab1">1</InternalRef>). Future research efforts should continue to investigate further expansion of breast conserving therapy.</p>
