<p>We present the case of a 42-year-old woman with a history of renal transplantation who developed severe enteritis while taking enteric-coated low-dose aspirin. Despite signs of localized peritonitis, conservative management—including aspirin discontinuation, fasting, and antibiotics—was effective, and surgical intervention was avoided. Endoscopic and histologic findings were consistent with drug-induced enteritis, and no infectious etiology was identified. This case highlights the potential for serious gastrointestinal injury from low-dose aspirin in immunocompromised patients and underscores the importance of early recognition and intervention.</p>
